answer to the second question, 2 is the answer to the
third question, etc..
Make the subject line look like this: Subject: Survey

2. (1-5) 1 is passive, 5 aggressive: what would you consider your
personality
type to be?
3. (C/D) Do you prefer to play (c)ooperative or (d)eathmatch?
The rest of the questions in this survey pertain to Multiplayer Quake
Matches:
On a scale from 1 to 5, 1 being Less and 5 being more,
after playing a game, do you feel more or less...
4. ...tense?
5. ...aggressive?
6. ...tired?
7. ...angry?
8. ...bored?
9. ...irritated?
10. ...frustrated?
11. ...calm?
12. ...happy?
13. ...dizzy?
14. (Y/N) Do you feel anger toward the winner of the game?
15. (R/B) Would you say the game (R)eleases anger or (B)uilds it up?
16. (S/F) Do you most enjoy the (S)trategy or the (F)ast Paced Action
involved
in the game?
17. (1..6) On a scale from 1-6, rate your skill level at deathmatch,
with 1 being
a novice and 6 being a Quake master.

19. You can type any comments at the end of the submission which you
